Can anyone tell me what’s going on with this weird blotchy spot just showed up today I’ve had him for about a year now. No issues other than this, eating and swimming fine.

Looks like an injury that has a bacterial infection. In tank, you can try Ruby Rally pro which is mild but best in Quarantine treating with Seachem Kanaplex

Looks like a bacterial infection, possibly from a bump injury. You would need to treat that in a isolation tank with a broad spectrum, gram negative antibiotic (the Kanaplex mentioned, or Neoplex). The issue is that catching the fish up and putting it into a smaller tank adds to the stress and may expose it to poor water quality. The alternative is to watch it closely and if the lesion doesn't grow any larger, just see if it goes away on it own.
