The coral looks like montipora setosa. Looks like hair algae attached to a dead spot on the coral.

First off, welcome to R2R. The coral looks like a encrusting montipora, but I'm not positive.

The stuff in the middle looks like green hair algae. Let's ask the #reefsquad

Not necessarily. If the dead spot is enlarging it could be. Monti's are pretty hardy as SPS go, just keep up with your water quality and keep an eye on it.
