Please Help My Anemone

I have a DayGlo Anemone. Starting about 2-3 weeks ago she started expelling zooxanthelle and/or waste. The reason I say it was zoox. is because I can see color missing from tentacles and oral disk area. And she has just been heading downhill since. I have been feeding her and that doesn't seem to be helping any (I would feed her every so often some brine/mysis. Once she started looking bad I tried to feed some clam bits, she didn't really like it, so I have gone back to the brine/mysis almost every day if she is not completely deflated). I tested yesterday and got- API; pH 8.0, Ammonia ~0-.25 (I can never tell with this, but it looked closer to 0), N03- 10-20ppm, Phosphate- .25-.5. Salifert; N03- ~10. Salinity is at 1.025. I have 2 LED fixtures from buildmyled.com on the tank. The 'nem hosts one male clownfish and a porcelain crab. As I look through photos, she started on a downhill around the start of the year. This anemone used to GLOW, but she became duller and duller, and then started expelling zoox.
I just don't know what to do. I don't want to lose her!

What type of water are you using? If it is RO/DI, what is the TDS?

Your phosphates are really high.

How long has your tank been set up?

Did you acclimate it to your lights?

Yes I am using RO/DI. Output TDS is 0.

Yes I know about the P04. I started using Red Seas N03 and P04 removal liquid this evening.

Tank was set up September 2012.

I can't remember if I did anything about the lights with her. When I put her in originally she went under an arch for a couple of weeks and the changed position to where she is now. So she kind of acclimated herself to the lights.
