I can't tell from the picture but I'm guessing that this is a new fish for you. Has he been flashing or scratching on the rocks or sandbed? It could be sick but my black cap likes to dig and excavate. His belly always is covered with white sand specks.

Yes, it's pretty new, he has been in my tank for about 1 month. He acts normal since i got it, no flashing ,digging , scratching or anything abnormal.
I'm not sure if this is some type of diseases or he just got injured from somethings else.

Since it's unknown, I would start with a broad spectrum such as Kana plex. If you think that it is pretty severe, you can treat with Kanaplex and Furan-2. No matter the severity, I would use Secondary as it will help the immune system.

I'm sorry i forgot to ask if it will be okay if i treat Kanaplex and Furan-2 in my display? Hes very fast and i think I won't be able to catch him unless i remove the rock.
Thanks

I wouldn't dose either one of those directly into your DT water. You could soak Kanaplex in food, if you also use Focus as the binder: https://www.reef2reef.com/threads/metronidazole.298762/#post-3655831

Food soaking isn't as effective (for external infections) as dosing the water, but it still can help if the medication makes its way into the bloodstream.
