Please help my Tridacna Crocea!

I've had her for 2yrs without issues. Placed higher up cradled in an old clam shell. No changes in parameters, flow or temp. No fish or inverts bugging. But the bristle worms seem to love hanging out under her.
Today I noticed she let loose her hold on the shell and was listing to her side. Closer inspection finds 5-6 bristle worms under her and even a couple inside where the foot should be. I found this thick white chunk and I'm thinking it's her foot :( She's open still and seem's ok but I've never experienced anything like this before.
Would the bristles cause her to lose her foot? Or were they simply eating that which was already dead?

She's closed up a little tighter then usual but then again I just got done moving her a bit to clean out under her. She is light reactive.
This is what came out from under her. It wasn't attached to anything.

I had a similar experience with a maxima. Was thrilled after learning they grow new glands.
