Please help what is wrong with my serpant star?

Hi everyone I hope someone can helpe. I habe hadarine aquarium fpr over 2 years. It is very mature I habe 2 other serpent that are perfectly fine, growing gigantic. I bought one the other day and noticed this today. Are these holes normal. I feed my Serpent stars weekly and he had a couple of small clams. Water is perfectly fine, my aquarium is 580 litres. Please can someone help, thank you!

I don't think that's normal, perhaps physical damage from something? If it's acting and eating fine I'd give it time to recover
