Poly Dying Head by Head!!

Hi All!
I need help!!

Over the coarse of about a week my poly heads have been dying off. I wasnt sure if it was a change in lighting but I’ve noticed the heads get weird spots on their heads... turn a fuzzy color and die.

Then brown tanks over as they turn into a nub!! I am not sure what’s going on or how to stop it!!

How did you acclimate ?
Feed it 2-3X per week, moderate light and flow. Looks like you can add a little white.
Dip it in Iodine. If none, try peroxide
