Polyp bailing out

Looks like my frogspawn polyp is bailing out. It has lost a couple tentacles this morning and been bunched up for over a day. Question is if the head does come off the skeleton, which it looks like is about to happen. Do it remove the skeleton and head from tank? I have read very unlikely that the head will survive.

I wouldn't say high flow. But I did move it to a lower flow area in case. It currently does look like the head is peeling up with some flow.

The flesh is receding but I dont see this as a bail out, at least yet. What are your parameters? Is it in a high flow area?
I did have a spike on nitrates a few days ago with it at 40ppm. My tank is usually around 15-20. Also, this is a new coral to my tank. He was placed in tank 7 days ago. I have turned off the wavemake for now.
 
I've never experienced this to be honest, so I am unsure if it can reattach. My assumption is that it would over time, there's a thread of someone regrowing a polyp that completely bailed out as well on here somewhere.
