Polyp bailout

Has anyone experienced a Polyp bailout? my frogspawn bailed, found one head under a rock (that's why he's so bleached- had no light for a few days) and the other on the sand bed. Glued a piece of plastic to the rock and placed them in, they seem to be doing ok, opening up during the day and closing up at night. Any suggestions on keeping them healthy would be appreciated it. Hopefully they will will regrow the skeleton in the future

Generally a sign of coral stress, too high of flow or parameters off such as alk, CA, salinity or ph related
