Nothing new added to tank in 8 months. My big black clownfish (percula) started hiding under rock last week and I have not seen it eat. Noticed one eye is popping out on left side and what appears to be ich looks to be appearing- first case. Tank is 2.5 years old and stable conditions except alkalinity 1.024-1.027. None of the other fish appear to be infected: blue tang, yellow tang, fox face, blonde tang, and a few other.
Tank is 220 gallons Trident dosing mixed reef. Jump in alk during water change.

Yes, the left eye looks bulging. Could be an injury.
And I see the spots on the fish's forehead. The spots are troubling. First thing that comes to mind with clowns is Brook. Some clowns have been known to be carriers and not show symptoms for weeks, months until some stressor event. Early stage Brook and Ich can appear similar. Can we get some pics that are a bit clearer? They are a bit fuzzy on this end. Any other symptoms? Breathing OK? Twitching, flashing, eating, etc.?

breathing seems a bit rapid no flashing and I haven’t seen it eat at all for a few days but do not see any thinning

Hiding and rapid breathing, loss of apatite can be symptoms of velvet. But you said nothing new added in 8 months. Nothing "wet" like corals, rock, sand, inverts?

6 months ago a harlequin shrimp. Have not seen it since I put in and I don’t see any dent in stars
