Porcupine puffer needs help

Trying to figure out what’s going on with my porcupine puffer. I always see him running the tank the last day he’s been laying in the cave and I noticed one of his fins are stuck to his body and he isn’t using it when swimming. He still eats. It looks alittle swollen we had a outbreak with ich about a week ago. I gave them metro pled and treated with Paraguard prior. Didn’t use medications at the same time. He’s been down for a day now can anyone tell me what this is from? Only pictures I could get not discolored

The Ich parasites burrow underneath the fish's skin and their feeding upon your fish can cause a bacterial infection. Consider treating the fish in a QT with a wide spectrum antibiotic like Kanaplex, Furan2, or Sulfaplex (aka triple sulfa). Keep in mind that antibiotics don't "cure" anything; what they do is give the fish's immune system some help and time to begin healing. It's also important to give the fish a variety of good foods. Adding vitamins like Selcon, Zoecon, etc. Also adding Beta Glucan to the food has worked extremely well on boosting a fish's immune system.
