Possible cause of death?

Hello all Had a death Saturday am, yellow Eye Kole tang divers den 3.5 weeks in tank then 48 hours later 2xRoyal gramma, edit gramma was 6 months and full grown
Tank us 75 gallon dot with 34 gallon sump refugium
Ammonia 0
Nitrites 0
Nitrates 40
PO4 ,1
Ca 400
Mg 1350
DKH 9
Temp 78,2 inkbird controller
Salinity 1,025
First loss fish distressed hovering just off sand coloration muted 9 hours prior eating swimming looking good, fresh water dip no evidence of flukes or stuff coming off fish into separate tank with bactashield died shortly after.pic below
48 hours later royal gramma being eaten by cleanup crew. Sry no pic but nothing of note visually, prior to deaths no abnormal poop or behaviors that I can tell.
Hopefully pic of tang is good enough for someone with more exp. to point to a cause. I’m hesitant to stress out remaining stock without an idea of what I’m treating for.
I feed frozen foods sometimes fresh chopped clam fortified with vitamins and small pellets usually soaked in vitamins as well.

What other fish are left? With 3 deaths that quick barring an ammonia spike I’d be concerned about Velvet.
 
Velvet was my first inclination as well but unsure as I couldn’t see any indication on the fish 9 hours prior to death of dots coating etc.

1 spot foxface , 2 spot cardinals, blue green chromis, blue chromis, Molly Miller Blenny, Maroon Clown, Possum Wrasse cleaner shrimp assert clean up
 
Any quick breathing? Swimming in to powerheads? Hiding from the light? Dusting or small spots? Blue light often reveals spots better.

I’m inclined to think likely velvet, especially if you don’t quarantine.

Were there any behavioral symptoms? Scratching, flashing, swimming into powerheads etc?

Typically it's something pretty virulent if you lose 3 fish quickly like that. Especially if there weren't any ammonia spikes or sudden changes in any parameters or equipment failure.

Unfortunately no signs at all. Been on vacation pretty much staring at tank and all has looked normal.
I tested ammonia RedSea and have back up badge side of tank haven’t seen any bumps in any of my numbers.
No new chemicals in house nitrate reactor effluent goes to skimmer to oxygenate last additions just over 3 weeks ago.

The position of his mouth stands out for some reason to me. Is it possible he choked on something?
 
So minus any signs of behavior changes flashing scratching and absence of visual spots and velvet look to skin would I still treat for velvet? If I’m going to stress the guys left want to be sure I cover all bases in a logical order. After death will signs of disease wether velvet brook or other still be visible on carcass?
 
No more deaths yet. Only one fish with a scrape on top of head but swimming and eating normally, this guy has always swum Mach 5 thru rock work
PO4 by new Hannah checker shows 0 not sure that’s correct need to rerun tests. Gha is sloughing off rocks and coating loosely everything as it looks to be dying now,
Still unsure what to do as far as attacking a root cause. Will post pics for folks if I see anything to try and help nail down what I should do look at etc
