Possible sick porcupine puffer

Hi. I just started working at a salon and I noticed that the puffer looked bad today. He hasn't moved and earlier I noticed gravel stuck in his mouth. He's following my finger with his eyes but he just seems hunched and lethargic. I took a picture any info would be appreciated.

First of all, welcome to R2R! :)

Who's taking care of the tank? A full run of water parameters are needed for something like this. It could be anything from poor water quality, poor diet or disease.
 
Some puffers can be finicky eaters. Like stated above water parameters should be checked. He looks a little thin to me but it could be the picture. I have heard of trying ghost shrimp to try and make them eat. Hope that helps some
 
It is possible that the puffer's teeth are overgrown. If this is the case, the puffer might not be able to eat.
