Problem with torches

Okay so this is the second torch I've had in my tank, 1st one the same thing happened. Was looking really good for a couple weeks and out of nowhere just started shriveling up and receding into its skeleton. I have a frogspawn, hammer, and elegance along with other coral that are all doing okay. It's only been torches I've been having problems with.
Parameters are as follow:
Alk: 8.1
Cal: 430
Mag: 1350
No3: 10ppm
Po4: .10
Ph: 8.0
Salinity 1.026

probably too late but if thats a powerhead i see at the top right of picture pointed directly at it was more than likely what killed it

What test kits are you using for nitrate and phosphate?
