Puffer in need of help

hi all- I have had my tank up and running for 7 months now and recently my blue spot puffer has something funky going on. His one cheek puffy and not sure why it would be doing this. The one cheek is for sure sticking out more than other side and has never been like this before. I have had the puffer for 3 months now. Is it a fungus? Any thoughts?

Don't know about the cheek thing, but he looks a bit different than mine. I think mine has a few blue stripes along with the spots.

Did he get stressed and inflate by any chance? Sometimes it takes a while for them to deflate.
