Purple tang dead. Help identify cause

So yesterday I did some massive cleaning of the system. This morning I woke up to one of my purple tangs dead . Attached are some photographs. Recent events , added f conchs to help with sand bed. Cleaned up the system. Note. Was healthy and no signs of any issues. Fat as can be. I have been having some issues with phosphate abs dosed 3 ml of seaklear into the skimmer. Skimmer is in sump which is basement. I am hard pressed to think it was the lanthanum chloride. All other fish doing well. Eating as usual. I do think I have some worm issues as some of my wrasses have large guts bombs. See the pics attached. Water parameters normal. No amonia ect
 
Here are the pics . I have included a pic of my mystery wrasse. His stomach has been like this for months. Crazy. I had a blue star like that also but recently . Last week he died. When I dissected him. It looked like he had a piece of gravel in his belly. Humble and all let me know your thoughts.

How long have you had the Purple Tang?

The white blotches on the body look suspect.
 
Purple tang was one of two. Had him for over a year. Maybe two.
 
I’m probly wrong but don’t red gills indicate ammonia burn?
 
I did a clean up of the sump. Scrapped up some.algea pff the rocks in the large sump. Algae and cleaned up rock. I tested for amonia when I found him in the am. Nothing

Did you clean the rocks with any utensils? Wondering if something on the tools may have contaminated the water. Or possible the cleaning disrupted the bio load

Whenever I hear of tangs dying after the tank was dosed with LaCl, it always makes me go that route. It could have been that, combined with another chronic issue, that would account for only one tang dying.
I presume the discoloration was after death?
Does the mystery wrasse have cloudy eyes, looks that way in the picture.

Hi all. That is how the tang looked in the am when I found him. The other tangs and fish are doing ok. Except for the belly on the mystery wrasse. That has been going for months.
 
I am thinking it was the lanthanum. I had been dosing phosphate -e from brightwell. No issues. Dropping into the skimmer injector. I the saw a post on seaklear which is a high. Concentration of lanthanum. I literally pout in a few drops in the skimmer. I am betting the resulting flucant made its way fro the basement where the sump is up to the DT upstairs and ended up in the tangs gills. I have no refrance photos to look so just a guess
