Even with no other livestock in the tank, diseases can still be introduced with new fish that can be hard to eradicate in a tank with calcareous materials, such as rocks and sand.
These diseases or issues may live undetected until you add more fish/corals or add other stresses to the system.
Quarantine should be for all new additions (technically even coral/invertebrates/"live" materials) if it is to be effective. Any living additions can be disease vectors with long-term effects.
