Abnormal tentacles and often bubble up oral plates are often result of chemical builds up (IMO) Are you changing your water regularly?
I often have anemones developed abnormal features like that when I let my tank neglect, improve when I take care of my tank again.

it is not the macronutrients like nitrates and phosphate, but micro-waste that built up in the tank again this is IMO. I never send water for indemp analysis. I know the remedy and the likely cause so I just don’t neglect my tank.

Anemones are intolerant of chemistry changes and often do this with water changes.
They need moderate light and water flow and too little light and water flow and they often shrivel up
