Qurantine Tank Questions

1. How do you go about setting one up? Do you buy a 20 gal kit? Does it have to have live rock to? etc.
2. When you put a new fish in a QT do you do anything to it other than just watch it? Do you go ahead and medicate the tank to make sure the fish is disease free as a precaution?
3. what is a good size for a QT?
4. Any other help or advice will be appreciated
