Nothing can be determined conclusively without white-light pictures and more history (i.e. how long have you had it - how was it acclimated, what is the salinity from which it came and now? could there be an injury. White pictures definitely required. I assume its only one eye

Two possible diagnoses would be: a simple eye injury, or Neobenedenia eye flukes. Generally, Neobendenia causes BOTH eyes to become cloudy, with raised portions, and an injury usually only involves one eye, but of course, that is not set in stone. Early on, Neobendenia can be in just one eye to start with.

Yes - those help. I think the eye issue is an injury, not Neobenedenia. However, there seems to be a number of tiny discolored areas all over the fish's body - those could be a different type of fluke. Do you ever see this fish flash/scratch?

Yes - just provide extra aeration for three days after each treatment.
