Raw chewed looking lips with hole in fin

My yellow eyed Kole tang has had a list of issues and I cant get a good photo of him. He came in from online retailer with a tear in his fin, but that healed quickly then he ended up with a hole in his bottom fin. in the DD photo he looked to have a pinspot but i assumed it was a lens issue. It was a pinhole tear when I saw him in person but I believe that healed on back fin. Now his lips are pretty raw looking like he hurt them or something really rough chewed at it. Nothing in tank with him. I haven't treated him for anything yet. Any ideas???

Is he picking at the rocks alot? What other fish are in the tank? Are you feeding it algae strips? Could be from fighting or picking at the rocks to hard looking for algae

could he have tried to pick something off the powerhead? he is quarantined with basically nothing in there. Would this be some sort of water quality issue or parasite or fin rot? His mouth had almost like dark red spots that were very very small. like it had been hurt.

i think it was ich but i still dont understand why he has a small hole in his bottom fin! its still there and hasn't healed.

Tattered fins can be a symptom of flukes have you done a 5 min FW dip?

it ended up being fungus and treating it for fungus worked. i did do a freshwater dip. i treated though for fungus and baterial infections for the same time, so that worked.
