RBTA dying/not eating

I've had my RBTA for about 3 months and it has never looked good. I've tried feeding it mysis and clam strips but it never takes anything. Now it is shriveled up. Is it dying?

A couple of questions. How old is your tank. What are your parameters? KH, Ca, Mg, NO3, PO4. What lights do you have? And what is the PAR that the anemone is under?
 
My tank is about 8 months old. KH-8 Ca-420 NO3-0ppm
PO4-0.25. I have 2 AI non HD 52s and 2 T5s
