My queen angel has lost some weight and color over a course of several months from not actively eating like she used to. I've started to notice her gills is slightly red but no rapid breathing. She is out and about cruising the tank as usual.

Is there any course of action now before it gets worst?

I'm not sure what's going on here - the fish looks pale (which would account for the red in the gills showing through the gill plates) but that overall pale, all yellow color is not normal. Given the time frame, this is some sort of chronic issue. One major chronic issue with longer term captive marine fish is Mycobacterium sp, fish tuberculosis. That can cause pale coloration, thinness and a general "unthriftyness" - that is, unfortunately, not treatable.

I think more background information could help, this post explains all that:

Please post video with you tube version. This one does not show image. Any sudden darting across tank, shaking its head or scratching against surfaces?
