What are some potential sources of pollution in a reef tank, and how can they be minimized?
 
I think technically to be a pollutant something just has to enter the water that is harmful or poisonous.

From observation (going anecdotal here), that things like lotions and aerosols tend to be the most common. So make sure you don't put your hands/arms in your tank unless they're clean and don't spray anything near your tank that doesn't belong in the tank.
 
Candles, air fresheners, cleaning sprays like windex
