Running a 10 gallon refugium on 50 gal DT. Refugium has matured sand, 3 inch deep. Also placed one snail (for now). However, should I put my sand sifting star in the fuge?

I would not.. if it ever gets the idea to explore it might end up in your return section and foul your pump. I have hermits in my fuge and they somehow get into my return section and they don't have sucker feet.

My return suction is blocked. But it does run light 24 7.

I have sponges covering flow to pump. Pump has a grill cover, sponge in that. I can put the pump in a media bag.

I'm more concerned with the constant light.

I feel like sand sifting star fish need are difficult to keep even in a main display tank, let alone a 10 gallon refugium. There might not be enough food in the fuge for it to servive

Refugiums are really a place to encourage naturally spawned critters without the fear of predation, so I wouldn't personally add anything in there. They should fill with life quickly enough and you'll have pods and worms springing out of nowhere.
If you want to add more fun things then I consider something you couldn't keep in your display, like a mantis, but not if your running 24/7 lighting.
