Where do you recommend? I tried some from eBAy but got 0% growth/survival. I'm not sure if I got scammed or just had a bad batch. And I ordered them last spring when temperatures were good for shipping.

Glad I haven't put in an ebay order. Where did you keep them?
Plastic shoebox on my workbench in the basement fish room. Temp might have been a little higher than ideal (low to mid 70's), but it should have been good enough.

But we're getting a little afar from my original question: who is a reliable vendor for whiteworms?

No, I haven't. Reviews are mixed. For the people that gave this a poor review, I'd expect to receive relatively few worms, but I also expect that the "culture" will grow quickly.

I have ordered from mad river seeds in the live food section of aquabid. Lareg quantity and excellent quality. All of the live foods I have ordered from aquabid vendors have been excellent.

Hah! That's the vendor that I used. Maybe it was something that I did. It's entirely possible. I'll add that I didn't see any adult worms in the culture that I received.
