Long story short, I haven't kept up with my RODI. I got it secondhand and was told the filters were all new. DI resin slightly color changed, but had some life in it. Jump ahead a couple of months, and I'm getting dinos, then cyano, then dinos, then cyano (repeat cycle like 10 more times). I finally decide I'm going to tear everything apart, clean it, get new sand, and just start fresh. I cleaned my rock, but admittedly I didn't acid wash because I wanted to keep the bacteria and I was frankly scared to nuke it. I restart and everything is going well for the first couple of weeks, and then the cyano and dinos come back. When I went to look for my TDS meter I realized, first, that I lost it, and second, that I had not checked my RO in so long it was embarrassing.
I've ordered new media and a TDS meter to check and see if I'm making poor quality RO, and to change the media anyways, but I'm almost sure that's the one stone I left unturned. I'll circle back once it arrives but just wanted to post this in case anyone is struggling to keep these (and GHA, had that too) under control. While writing this I also thought to have my water ICP tested as well for silicates or anything else I might be able to address with an add-on to the RODI system.
