Rhodactis dying or splitting?

I’ve had this trio of rhodactis (green base with orange polyps) for a couple weeks now and one of them just melted off in mere hours. While one of them is doing excellent, another one is showing signs of...well, abnormality. They’re around 3” fully expanded. As for the abnormal one now, the mouth area is spewing with wirey tentacle-like stuff. Is it melting as well or splitting?
 
water params are quite okay, sitting around 10-15 ppm (test kit doesn’t specify).
running led lighting, par isn’t that high prolly not above 150 at the very top.
the rhods are at the very bottom of the tank, with new mushrooms growing out around the sides.
tank’s been running over a month now with xenias and gold clove doing bery well

might have to pull up the salinity though...it’s measured around 1.022

There is more than one parameter that you need to monitor for corals, I assume this one is nitrates? That is fine. Now what is Alkalinity, Calcium, and Magnesium? Salinity should be 1.026 as well. That definitely doesn't look like a happy shroom.
