Rip blue tang - advice please

so my blue tang has sadly just passed away, had this one for around 5 months, never really shown any signs of ich or stress etc

tanks levels seem to be ok, salinity been low last month but got that back up slowly and doubt this is the issue- snails crab and all other fish swimming around fine, nothing else spiked

First pic is the best one I’ve managed to get from a video I took on Saturday. Looks ok. Just found her laying on the bed, took her out and can see discolouring and also white markings on her black stripe

Could anybody provide any insight into what you think has caused this? Starvation? Attacked? Disease?

Hard to identify post mortem. Can you please post some clear white light pics of remaining fish and also explain more about your tank and quarantine methods?
