I've got the follow RO system given to me on loan, the owner doesn't have any manuals as it was purchased like this and never used.

I'm trying to install it.

My questions are
a) what are the 5 stages? as in when you look at it
b) I've got connections called "1 To Water" and "4 To Faucet"

Which one in the fresh cold water input?

Fauset got to your water supply. Drain is just that, a drain to get rid of the bad water, tank goes to the tank or water top off. I hooked mine up at my washer tap. Put a Y hose adapter on there and ran it to my ROunit. The drain is ran down to the washer drain.
 
That was my assumption too, what's the blue label for?
 
Typically the first stage (bottom left) is a sediment filter followed by two carbon filters, then to larger horizontal RO membrane followed by smaller horizontal charcoal filter (T33).
As far as connections go:
1. goes to your water supply
2. goes to drain.
3. goes to expansion tank
4. goes to your auto top off.
The last stage (T33) would normally be replaced with a DI filter.
