Could you capture him and do a freshwater dip for flukes? And then move to a quarantine for observation to see if he improves

Looks like Flukes 
exact same thing happened w my royal gramma. He survived hypo and it got rid of flukes. But he died shortly thereafter due to infection of the sites
Definitely do a FW dip to be sure it’s flukes. Hope he makes it.

I would definitely bet on flukes, if he’s positive you can either remove all fish to QT and treat with Prazipro 1 teaspoon (measured) per 20G and leave your DT fallow for 4 weeks or you can treat the DT with Prazipro as it’s generally Reef safe.
Aerate heavily remove carbon and run skimmer without collection cup if you treat in DT.

Thank you guys so much! Definitely QT'd before adding, it's been in there for over a year. Possible to catch it and treat in QT I'm sure... I have prazipro and am pretty close to treating the DT just to make sure I get everyone. I do have a melanurus wrasse though, should be ok right? Downside is I have quite a few bristle worms that it would kill off.

If you are treating your DT then yes you may have a large die off, a couple bottles of biospira could help that. I think the Melanarus would be ok as long as you don’t overdose and aerate very heavily.
If you QT you could avoid that die off but your DT will need to be fallow for 4 weeks.
@ngoodermuth any advice on the Melanarus?
Melanurus should be fine, just be careful not to over-dose. I’d be cautious if you have a ton of bristle-worms though... could cause issues if you have a large die-off.
