Why does it look pinched
Look at the size of the mushroom head and the rest of the body. When they stretch, it's the body that expands (not the head). My mushrooms absolutely (positively) hate being on the sand bed. It's ok to place them lower down but they want to be on a good-sized rock that they can expand and grow out on.
 
I see what you're talking about, the edge is pinched. My guess is salt buildup from the lid/edge of tank fell in and landed on it, if that's the case then it will open back up pretty soon. Shrooms pinch like that as a response to irritation, but the rest of the animal is open and healthy so I wouldn't worry.

EDIT: I bet the green hammer in the foreground stung it with a sweeper.
 
It’s been in the same spot for a while and how often should I feed it along with what level do phosphate I need and how to lower it
If feeding reef roids or over feeding will increase phosphate- adding a pouch of chemipure blue or elite will help reduction
Feed 2-3x a week
 
my hammer lived peacefully next to my florida ric for at least 6 months...and the minuite i went on vacation it started to sting it...after i moved the hammer 6 inches away it kept that sweeper flailing around for another month or two...it was longer than the regular hammers, colorless and had a round ball on the end instead of hammer shape...the ricordea shrunk back at least 1/2 inch on that side...all is good now but all its growth has been on the other side...if it wasnt glued it might have crawled away

those mushrooms will do that like other stated, they will also shrink and huddle up if they are unhappy with their placement, these guys typically like lower light

Also make sure youre looking at the temperament of the corals, light requirements and flow as well as the chemistry
