Sac underneath blue eyed cardinal?

Hi. Last night I noticed what looks like some kind of sac underneath my blue eyed cardinal. It looks like it could also be a pineapple sponge as well ‍? Thanks for the help!

Prolapse?

Is prolapse common? This appears to be perfectly white which leads me to believe it’s something else.
