I just bought a sailfin wrasse and I’ve been reading that wrasses are more sensitive to preventative medications and I’m wondering if I should just watch and observe the wrasse and not add any medication to the tank
If I treat preventatively what is the safest way to proceed ?

Lets start with the second question first: There are numerous protocols for treating wrasses with medication. I would suggest using a Chelated copper (such as copper safe). Second - Prazipro is also tolerated by most wrasses - BUT - there are lots of anecdotes about problems.

If you do not want to use medication (unless needed) I will give you 2 suggestions - 1) buy a wrasse that is pre-quarantined by a reputable firm. Then you will be sure its done correctly - and I would also observe even that fish for a couple weeks before adding to your display. 2). Observe the wrasse in a separate QT tank. for 4-6 weeks. Treat as necessary.

PS - no matter how you do it - make sure they are eating well. For flukes - you could also consider a Freshwater dip prior to your observation.
 
PS - you can also consider hypo salinity - but this is mainly effective for cryptocaryon.
 
depending on the issue at hand (eg: parasite, fungus, bacterial) some safe meds for wrasse are :

Fenbendazole for external issues like worms
PraziPro for flukes and other external
Formalin - covers a variety of issues

My Choice: ACRIFLAVINE - RUBY RALLY PRO WHICH TREATS:
- flagellates
- Marine Velvet
-Gill Flukes
- Clownfish Disease (brrok)
- Bacterial Infections
- Tail & Fin Rot
- Secondary Infections.
