Should we worry?

Hi All,
We have a mature 125 gallon softie tank and for the past few days our blenny has been hanging out with the cleaner shrimp a lot. He is also acting more agitated than usual.
There are no visible spots and the other fish in the system are all acting normal.
Should we be worried?
 
Have you noticed any scratching or rubbing against the sand/rocks?
What type of blenny is it?
Video doesnt seem to be working for me

It’s a sailfin blenny and he is scraping against the sand sometimes. Hiding more. We introduced a diamond back goby we’d had in a nano tank for several months - I’m not sure if these events are related.
 
Following, with him using the cleaner shrimp I'm betting on something on his skin but I'm not even close to an expert
 
Thank you. Hoping he recovers. He’s a great fish. But he hides in and under the rocks often so he may have been hurt by a bristle worm. We’ve had worms sting fish in the past.
