Sick black cap

can anyone tell me what this is. Came home feom a weekend trip and found him this way. Still eating fine and no heavy breathing. Thanks in advance.

What are your tank parameters? Does anything else seem to be effected?
 
Sorry I was trying to get to that. It is in a 20 gal qt tank with a mcoskers wrasse. The wrasse looks fine. They are both still eating well. They have been in qt for 2 weeks. I havnt treated with anything yet because i wanted to make sure they were eating well first. Ph 8.0 ammonia 0, salinity 1.025. Thanks again

Any chance the wrasse is picking on it?
 
Not that i have seen. If anything the black cap is the more agressive of the two. They both just swim around until you get close to the tank then they go hide. Unless your feeding then they show up. That is the only way i spotted this because it was feeding time. Usually pellets in the norning and brine or mysis at night.
 
It's hard to tell if its a bacterial infection or fungal infection of some sort. Or being picked on by the wrasse
I must call upon the power of the #reefsquad and @Humblefish for help on this one.

What sort of inverts do you have? Definitely looks more like a scuffle to me as well.
No Inverts. Just him and a small mcoskers wrasse. Never seen them be agressive with each other at all. Thanks for the replies I guess i will just keep an eye on him. Should i treat with anything. I have prazipro and metro from hikari on hand. Thanks again
That was my guess as well.
 
Does the other side look the same? He could've gotten his head stuck in something, I know it's a relatively unlikely thing to happen but I've seen it happen before.

No its just one side and a couple spots on his head. The other side is fine.
