So, Here I am with my first sick fish. I am/was planning to do a routine water change today and fed prior to doing so. I realized the fish wasn't coming out of the rocks to eat which is abnormal. I started watching for him and from what I can tell is one eye looks normal and the other is clouded over and swollen. Is this likely the result of a bacterial infection?

I have 2-300lbs of rock (was live but given to me dead, and I added 10lbs of live) in my tank and it is stacked on back wall so impossible (rookie mistake) to catch any fish without draining a fair amount of water and removing rocks so getting the fish will be an undertaking in itself. Without dragging this story out, the tank was given to me broken down and in rough shape, it is ~175 gallon acrylic tank that is in my garage bc I had nowhere in my home for it. Just getting it going and maintenance was more expensive than I expected, and I was given almost everything.

I am trying to decide now what to do. I do not have a QT tank set up. I do have another ~30gallon tank, but no components at this time. Do I basically break down my big butt aquarium to catch this fish and try to QT him or do I just let it ride and see if it resolves? I am enjoying the hobby but due to the size of the tank as well as it being located in the garage certain things have made the setup less than ideal. I decided 3-4 months ago that I definitely would not be adding any more fish due to fear of system failure and losing $100s in livestock. I have 2 clowns, 3 chromis, one foxface, one yellow tang, and a blue tang. No corals.
I essentially do not want to invest in having a back up chiller$$$, filter pumps, and fans for if/when something fails. At this point if any one thing goes out, I am not above replacing it but I just cant buy all the extra stuff for it to sit around. If the chiller goes out during the summer, the tank is toast unfortunately. Any help or recommendations would be great!

I had similar last week on my goby. I think, since he hides in a cave, he scraped it or a bristle may have gotten him. It took a couple days for it to heal. I also thought he had an infection but he didn't. Is he showing signs of distress other than beimg timid amd not eating as aggressively? Mine did not eat during this period either. Pics always help those with more knowledge as well as incorrect diagnosis.

It could be:
  1. Eye trauma. If so, dosing Epsom salt (1 tablespoon per 5 gallons) might help heal the swelling.
  2. Flukes in his eye: https://www.reef2reef.com/threads/flukes-–-general-guidelines.224423/ (If flukes, treat with Prazipro right in the DT.)
  3. A bacterial infection: https://www.reef2reef.com/threads/bacterial-infections.191511/
Only #3 might necessitate removing him from the aquarium, in order to dose antibiotics into a QT. However, many times food soaking vitamins will enable his natural immune system to overcome a mild, gram positive infection. If he's not eating, you can dose Vitamin C (reef safe) right into the water column which his body will absorb.

Thank you for your responses.

Trauma is certainly a possibility as they are regularly darting around in and out of the rocks. I currently feed ocean nutrition formula one and two as well as ocean nutrition green marine algae. Is this something that you think needs to be acted on immediately, or could I wait a day or two and see how it looks and hope he starts eating? I have free time today, not so much during the week. Going to to water change today as scheduled so now would be the time for me to grab him, or I could possibly put it off a few days. Do you have a type/brand preference on the vitamin C? I have been in vet industry for 15+yrs but never cared for fish other than maintaining a few tanks during college for the school. Thanks again!

I'm on my phone right now, so I can't see the white spot too well. Is it inside the eyeball or on top of it?

Humble will confirm but it looks as if it may be flukes. Medicatiom protocols should be considered.

It's not flukes. Lymphocystis (virus) is possible, but a bacterial infection is also very possible. Treating with erythromycin in a QT would be best.

Thanks Humble. I went ahead and did water change and that was how I was able to see him and get picture. I did not pull all of the rock out in effort to catch him. He is in one of the bottom corners of the tank behind the rock wall and just sitting very still clearly distressed. To catch him I will have to likely drain water and move rocks but I could try to get him out without doing all that. I was able to get him to move earlier but he is still lightning quick! I will keep everyone posted. Thanks for everyones input!

PM update: Saw fish in same spot sitting static against rocks in bottom back corner of tank this morning. Had to work all day, just getting home and fish is no where to be seen. My guess is he is laid up in the rocks somewhere so just kind of standing by. To give yall an idea of this "great" rock wall I will post a pic. It is actually smaller than it was after I removed some yesterday

Tank is 3' high. Morning update: didnt see him when I came out to check. Started AM feed and he came out and ate which I am taking as a decent sign seeing as I think the last time he ate was friday or saturday. He is kind of swimming around but I think he is basically blind in that eye so it makes life a little difficult right now. Going to keep watching him. Thanks again everyone for input! Hate to see him struggling and me not really having a great solution.

He seems to be doing better! He is out swimming around again and his eye is not nearly as cloudy as it was. Eating well at meal time so I think he is headed in the right direction!
