Been treating DT with Polyplabs Medic and Ruby Rally Pro for 2 weeks. This littlw guy looks like he has been run over. Head, dorsal fins and tail all appear to be disintegrating. Fish is not eating. Pulled him from DT into new hospital tank. Pic and video below.

What should I be looking to treat with?

Polyp Lab Medic rarely works and the peeling you see may be the peroxide salts in Medic impacting the slime coat on the mandarin. Mandarins do well with most meds with exception of copper but may be a reaction. I deem ruby Rally as safe and a wide treatment medication.
The fish seems to have either bacterial or fluke issue but Ruby Rally will address both, just takes a little longer.
Treat at 1oz per 10 gals
 
I would try something more drastic, personally. Prazi and copper. Maybe a FW dip first. That fish looks pretty crappy.
 
How long have you had the mandarin, was it eating before and what was it eating?
Got him from a local reefer who was breaking down his tank 3 months ago (he had it for a few years). Was eating flake and pellets for him and pellets plus pods for me. The appearance and lethargic behavior started about 3 days ago.
