what do you think is the issue with this guy? I have tried netting him to move to hospital tank to no avail. He can still move quick when the net comes close.

Looks to be flukes. Best plan of action is to
Conduct a FWD in a clear bucket and see if any opaque sesame seeds size pieces fall off him. Once identified you can administer Prazipro to fully eradicate the worm.

If you cannot catch him you can dose Prazi as it is reef safe.
