Can someone please tell me what might can be wrong with him? He has these blotches and his nose is abnormally concave. He is leaning sideways and not swimming much. His fins are really jagged and he wont eat nori or mysis shrimp. Can this be caused by high nitrite?

What is the background of this fish? We need much more info than a pic to help.
Was it healthy before? QTd? What tank is it in? How long this going on?

Looks like it could be a number of issues - Severe HLLE, Ammonia Poisoning, Uronema - but from the looks of that picture I doubt its going to survive

Hi welcome to the reef ,
sorry unfortunately i agree this fish is not going to make it .
how long have you had it?
any other fish in tank ?
any other info will help experts.

My opinion is that the fish is extremely stressed and starving. The sunken in cheeks is an unfortunate death sentence. Add the fin damage along with the behavior and it is almost certain. Sorry.

He ate thursday then stopped now past 2 days have been going way downhill

No tang is gonna survive long in a 20g and not QTing a tang is pretty much guaranteeing problems down the road.

stick to simple fish like the damsels/clown/firefish until you understand fish husbandry - it gets pretty challenging with marine fish.

not sure what 'yellowtail' is - if its a purple tang, get it rehomed asap

the sticky articles here are a valuable read if your new

Yellowtail damsel

agree 20 gal is too small for any tang, but not cause of illness,
best to remove asap,
A euthanize
B put in bucket of tank water with airstone till morning
be prepared to treat all remaining fish if more probs arise

Yea, nothing besides Jesus is gonna bring this poor tang back, id euthanize it.
You can do so most humanely with clove oil

And id find a new LFS

Your damsels may be comfortable with some decorations like snails or what appears to be a bubble screen in the tank, but please don't get confused, this is not fresh water and any moderate level marine fish will pay the price not having a tank in the best condition, you just confirmed it. Any species of tang and all the more reason an Acanthurus requires at least 70-90 gallons for more comfort, sometimes that falls short too, follow the advice of mates and stay away from more difficult and expensive fish in this hobby like tangs, angelfishes and above all butterflies until you have some experience in maintenance, read a lot and make notes, here in the forum there is a lot of information and little by little the doubts will dissipate and there will be security before pulling the trigger with another fish or with another type of project and don't be discouraged, that's how many of us started.
