Sick tang

Any diagnose for this?

Its not my tang but it was brought to me just now because I have a QT tank.

Side info;

Water parameters of the tank are slightly elevated but LPS does fine.

Fish has been in captivity in the same tank for years.

Yesterday a tank mate (sand goby), also in captivity for years died without symptoma.

Owner bought a new fish about two weeks ago from a LFS that is known to be a risk source for all the fun diseases.

I started by giving it a 150ppm H2O2 bath before QT tank.

Hopefully on of the others will answer more as I'm in and out. However, I would start with a video of the fish - It looks like a parasitic disease or mucus plugs, the fact that other fish have died suggests a parasitic disease. Is the fish breathing fast or any other symptoms? Is it being treated now (i.e. copper) - or just observation? @Jay Hemdal @vetteguy53081

Peroxide is not the answer and often a short cut that rarely works

The fish has been with me for a couple of hours now. I've put it in a empty tank with just flow/heater.

Fish acts normal just as any fish that would be going through dipping and placed in a brand new QT tank. Could be mistaken but I did think I saw it scrap its head against the glass for a brief second.

Off the cuff, I would say this is marine ich. I base that on the spots being visible on the fins as well as the body.

Coppersafe or hyposalinity are the best treatments for that.

I acclimated it over about half an hour, but the fish came from a mixed reef and went to a QT set up with water change water from one of my frag tanks. 35ppt

The fish has been in a mixed reef (not mine) for years. 34ppt.

29 ppt won’t be low enough to cure any disease, you need to get to 16 ppt for flukes and even lower for ich. I don’t have the salinity conversion handy, but you need a specific gravity of 1.009 for ich.

No need to drop slowly as you can drop quickly safely, but slowly when raising. For ich/velvet- 1.009 and as suggested by Jay for flukes

Correct, some strains thrive down to 4 ppt salinity, and that is barely even brackish water.
