Sick yellow tang

I have a 58 gallon reef tank for 20plus years. At present I have much coral a 5” yellow tang and a royal gramma.
my yellow tang keeps moving head up with gills flailing. Water change yesterday of 10 gallons. Noticed it a week ago. Thought it was getting algae from a rock. It gradually increased. Now is pointed up more now than earlier. No spots on tang

That looks like bad news, unfortunately. How long have you had the tang?

That is unlikely to be related to water quality if the invertebrates are all doing well. Can you post a video? (taken under white light is best).
