Six line wrasse died but why

So I got a six line wrasse and a rock flower for my 20 gallon tank. My rock flower is doing great. But unfortunately my six line wrasse died a day after, I’m not sure why this happened. All my clownfish are doing great and all my parameters are fine. If you have any suggestions to why this happened please let me know.

How long has the tank been running? Could you give us a little more information about the tank and exact parameters?
 
I get two months without issues, but the key is to know what your levels are. Many reefers say no issues till now, I used to say that, but the fact is you want to prevent another issue like this it costs money and we lose a life if you will :(.

Other than acclimating it comes down to your tank, and possibly if the wrasse was sick, did it eat before you purchased it?

I did not see it eat before I got it but it was not skinny

Calcium is 420
Phosphates 0.1
Nitrates .3
