Six line wrasse injured?

I woke up this morning, tank doing good, fish all fine
(2 clowns, goby, and wrasse)
Now the wrasse has a part of his scales by his gills that is pale, and is twitching every now and then

Backstory
All fish qt for 2 weeks, and I moved fish one by one into a new tank with live rock from fish store(no fish in their tank) and most from my smaller tank.

No new additions

Also, still scouring the rocks and sand for pods and eating them eats mysis like a pig
 
Most if not all 6 line wrasses ive had have this discoloration on their side.
You may have not noticed it until now.
Twitching may be from something else.

Wont argue there but I doubt theres an issue to treat since ive experienced this discoloration on multiple 6line wrasses over the years.
If he stops eating then i would start to worry.
 
I just don’t want him to develop an infection, so I guess I’ll soak food in garlic
