Got this guy about two weeks ago, he's alone in 10gal frag tank.
Third day looked like he scratched his side, white scratch, looked like it was healing, covered with skin nicely just a small bump.
Today I wake up, he's scratched much more, looks like he's siting on sand once in a while, and slightly turning on it's side, mouth open and breathing heavily.
Also had a ammonia jump overnight, didn't have time to test how much, did quick water change.

That looks like a bacterial infection has set in. You'll want to treat with a broad spectrum of antibiotics (Kanaplex, Metroplex and Furan-2 are a combination that has worked well) for ten to fourteen days in a quarantine system.

^^ Agree

Also ask yourself this: Why is he scratching himself in the first place? Could a parasite or worm be irritating his skin?

I agree with the guys, but I do want to point out that I see a lump just behind his pectoral fin. Is that a correct observation?
