Skin missing off fish's head

Can someone tell me what could possibly have caused something like this to his head, Maybe it was a rock scrap or something but I don't see anyone being mean to him other than some times the tang will chase him away from him but besides that the fish acts normal as if nothing happened and swims out in the open like he isn't afraid but I have no clue what happened and also if you have an idea how can i help him heal or is it just gonna be time that heals?

Looks like one serious secondary bacterial and fungal infection which happens over several days.

I don't believe it is one because the it looks more like a chunk missing from it but i could be wrong but i didn't see this wrong with him yesterday which makes me believe It might be like a injury but i am unsure. Because just cause i didn't see it doesn't mean it wasn't there.

An injury would be possible but one that is severe such as getting caught in a powerhead as an example
