Slumping toadstool

Hey all!

I have an unidentified toadstool in my tank, had it for almost a year after winning it at Reefapalooza. It came on a plug and I cut the stem off the plug and glued that to a 4×4 ceramic tile. It has never stood up straight , regardless of where in the tank I've placed him. Is there something I'm not doing that's causing this lean?

Also, any idea wgat kind of toadstool this is?
