I lost my first fish to carpet surfing. I was cleaning algae off the glass, had the top on one section off and didn't notice my ruby head wrasse jumped until I stepped on it--very lightly as I was barefoot and thought I stepped in cat puke.... I had moved to the tank beside the display, had the screen on by then, but didn't notice the fish on the floor. Must have walked over it a few times. It was still breathing but didn't make it. I put it in a pitcher of tank water and watched for a couple hours.

I loved that fish!

So bummed.
 
Sorry to hear :-(

I had a long nose hawk go surfing into my shoe, which sit beside the tank. I didn't notice until the next morning when I went to put my shoes on
 
So sorry.... but wrasses love to jump. I had a QT next to my DT many years ago and watched a Wrasse jump out of the QT and make it into the DT. Most unreal thing I’ve ever seen. I left him in the DT even though quarantine was not over. I figured he deserved it!
 
I learned that one the hard way too. I had a gold assessor that jumped when I was cleaning and doing a water change. I didn't notice he was missing til I found him crispy on the floor. I do a head count now every time I have had my hands in the tank.
 
Sorry to hear. Yep, Ive had my fair share of jumpers over the years. Knock on wood, haven't had any with my current system. Canopy has stopped a couple attempts though.
 
That’s a bummer. My male Lyretail Anthia went tile surfing two days ago. With the lights in the kitchen dimed I was futzing around when I looked on the floor and saw this fishy outline on the tile next to my cat! I actually thought for a second or two “did an animal cracker fall out of the box? Are there even “fish” animal crackers?”
Then it all hit “tank fish!” I scooped him up and he was “tacky” so he’d been out a while. Who knows what the cat had been doing to him. Without much hope I snagged him in the tank and swished him around. About 5 seconds later he zipped out of my hand and into the rock work. Right as rain so far. He’s missing his “lyre” but it will grow back.
Very sorry about your Wrasse, but had to laugh at the cat puke comment. That is some nasty business. How can such a cool and regal animal create such an undignified pile of goo?
 
I recently lost my blue hippo tang. One day I noticed a fishy smell in the room. I took inventory on my fish and noticed he was missing. He was behind the tank. Not hard to find a 4 inch blue fish. Lol
 
Lost a beautiful orange shoulder(I think) wrasse had the cover off for just a moment ! Sorry for the loss! Folks ,always keep the tank covered! It's the best we can do. They might even get thru a tiny opening.

My long nose did the same one day.. I was feeding the tank and I heard a plop..
Couldn't for the life of me figure out what that noise was..

Luckily my dog knew what it was.. and was sniffing and looking inside of this old bucket of dirty water I had next to the tank..
Here... low and behold.. my little long nose was chillin in that bucket of nasty cold water... he let me pick him up by hand and dropped him back In the tank unscathed..though you could tell he was a bit confused on what just happened
 
"Involuntary fishslaughter" is a Level 13 Misdemeanor in most States. Go sit in the corner with your nose against the wall for 15mins

I added a male pink tail fairy wrasse a few weeks ago and planned to build a DIY fish screen. Well, didn't build screen right away, and the wrasse went floor surfing twice in two weeks. Fortunately I was home both times and put him back into the tank. The second time he looked rough once back in the tank, thought i would lose him. Ordered the DIY screen supplies the next day. Built the screens and everything is fine. Guess I was lucky.

Sorry for your loss. Now please always wear a rubber soles shoes when working on or putting your hands in the tank, this could save your life.
