I just don't get it, my small toadstool died and now it looks like my hammerhead is ? I've had it for about a full year, changed salt recently from seachem to instant ocean and increased filtration, other then that keeping up with 1-2 week water changed, salinity temp..

Are you running carbon. Softies have toxins and when they die release them.
Have your perimeters been stable alk cal mag no3 po4, if not testing test every few days when things get wonky, just a good plan, and test once a week for at least alk and cal. Temp swings? they don't like that too much either.
Sorry for being Cpt obvious but just some things to think about.

Nah wasn't captain obvious at all there is alot I don't know, outside of ph,salinity amonia and nitrates I don't test the rest, it's a small 20 gallon tank, I just assumed weekly to bi weekly water changes would be fine, as far as softies releasing toxins I had no idea, does carbon get rid of it ? And can you use too much ? Because I have a good size bag in the back sump. I don't dose anything and I know the lighting is good because I use 2 aquaknights, the temperature is definitely solid.

Ok, yea carbon will help absorb remove the toxin, but it dosen't last very long, gets pluged up with bacteria. I would try a water change to help get rid of any toxins released by the toad.
Also how long was the dead toadstool in the tank before you could get it out. How long after it's death did the hammer show signs of stress?
You are probably ok with water changes taking care of every thing but as coral grow they consume more alk and calcium, well lps and sps do softies don't use that stuff. Coralline algae will however use alk and cal.

Tbh I feel maybe throughout the week it was slowly going downhill and I was hoping it would come back, yesterday I checked and it had like a brown detritus on it ? So I chucked it. Hamerhead started not extending like 2 days ago? And looks white ish at the bottom. Ughhh I feel like everything went downhill from 2 weeks ago, my midas was very thin with my knowledge I think he had flukes and the only medication I could treat it with in Canada is herbtana, was supposed to be reef safe. Blenny died 3 days after then the toadstool was looking bad....snowball effect it feels, sorry big text wall

From the pics I an seeing there isnt much flow ? xenias in the background look still? drooping tentacles on the hammer makes it look like its stagnant as well?

Yup something went wonky. I would do 20% or so water changes 1 a week for sure maybe 2 a week since it is a small tank until say 80-100% of the water is changed out. Stability is the key so not too much and not too fast but need to get what ever is causing the problem out. If you use carbon put it in a high flow area but not too high the carbon will bounce around and grind into a fine dust, just a heads up. I use a diy reactor plumbed off the return pump for carbon GFO.

White like this is good.

yeah it looks like the bottom of yours, but it has some openings or holes at the top that ive never noticed before like its splitting.

Thanks alot for everything guys, ive set my power head in a better area so the flow will push around more , and tomorrow i will be doing a big water change and give the carbon a good rinse. it has just gotten a little discouraging, lose a fish then a coral, then another coral going downhill... and in the last few weeks ive been slowly purchasing everything to upgrade into a beautiful low tech 75g with t5's keeping it as simple as possible......softies only and a few fish i love. Aquaclear 110, 2 wavepumps, fiji pink substrate... real simple.

I agree that flow is important. What is your set up? You have 20g, filtration, pumps, powerheads? knowing all of this will help.

IM nuvo 20g tank, 2 RFG nozzles with a sicce 1.0 pump pointed at the surface, a small koreli 240gph wavepump, and i slapped an aquaclear 20 on the side for extra surface agitation and filtration.
