Something in my candy cane coral?

So about a week ago I fed my lps. One of the heads of my candy cane a long red about 1 inch long worm (literally looked like a piece of red thread) was protruding out if the polyps mouth. Today I get home from work and notice the head is in bad shape. Do I frag the head off? Qt the whole piece or what? Photo taken 8am before lights on.

I would leave it alone unless it looks like it is spreading to other heads. Each heads a separate animal so it is not uncommon for one head to die off.
Can you tell us more about you system, it looks fairly new.
 
40b been up for 6 months. Added 2 extra pieces of dry cured rock late. 15 gallon sump with fuge and rubble.

This coral came out of my 12 gallon aquapod, had it for over a year from just a couple heads.

Fish -2 occ clowns, 2 inch foxface, 1 YWG, falco hawk, and usual CUC.
Corals- 1 RBTA, hammer, torch, monti plate, birdsnest, hydnophora, Duncan, gsp, palys
 
We recently moved it due to hydnophora being an aggressive coral. Never made contact with sweepers.
 
Could have eaten something it didnt like. I had a duncan spit out a live bristleworm once. Freaked me out but the coral was fine.
